COORDINATOR WILL VERIFY THE NEED FOR FINANCIAL OR APPOINTMENT TRANSPORTATION ASSISTANCE.<\/p>\n\n\n\n<p>3. IF NEED IS VALID, COORDINATOR WILL NOTIFY REQUESTER OF THE LION\u2019S CLUB LEVEL OF ASSISTANCE.<\/p>\n\n\n\n<p>4. LIONS COORDINATOR WILL SET UP ASSISTANCE AT ONE OF<br>CHESTERTOWN\u2019S EYE CARE PROVIDERS.&nbsp; (DEPENDABLE EYE CARE,<br>SEIDENBERG PROTZKO EYE ASSOCIATES).<\/p>\n\n\n\n<p>5. REQUESTER WILL MAKE AND COMPLETE THE APPOINTMENT WITH<br>CHOSEN EYE CARE PROVIDER FOR THE SPECIFIC SERVICE.<\/p>\n\n\n\n<p>6. LIONS MEMBER VOLUNTEER WILL PROVIDE TRANSPORTATION TO EYE<br>APPOINTMENT IF NEEDED.<\/p>\n\n\n\n<p>7. LIONS WILL REIMBURSE THE EYE CARE PROVIDER FOR THEIR COMMITTED FINANCIAL ASSISTANCE.<\/p>\n\n\n\n<p>8.
